About the Facility Cordova Wellness & Rehab Center is a 242-bed skilled nursing facility offering short-term rehab, long-term care, and outpatient therapy services. Residents enjoy parks, recreational facilities, shopping, dining, and cultural activities, making Cordova a comfortable and convenient place to work and live. ⚙ Responsibilities
Review clinical information and assess its impact on the assessment and treatment process. Conduct comprehensive functional assessments using standardized procedures. Develop and implement individualized treatment plans with achievable functional goals. Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly. Revise treatment plans based on patient performance and progress. Supervise and guide therapy assistants to ensure proper treatment techniques and the delivery of care. Educate residents, families, and caregivers on exercises and therapeutic procedures for carryover. Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes. Assist with discharge planning and maintain knowledge of community resources. Participate in care conferences, meetings, and quality improvement initiatives. Maintain clinical competence through continuing education opportunities. Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines. Support the company's mission, vision, and professional expectations. Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
Qualifications
Bachelor's or Master's Degree in Occupational Therapy BLS/CPR certification (current or obtained during orientation) Strong clinical reasoning, communication, and documentation skills Ability to collaborate effectively within an interdisciplinary team • Benefits
