The Role The Physical Therapist is a licensed healthcare professional responsible for comprehensive evaluation, development, implementation, and oversight of individualized physical therapy treatment programs. This position is classified as Non-Exempt and reports to the Director of Rehabilitation and the Facility Administrator. Core responsibilities include:
Performing comprehensive evaluations and reassessments in accordance with physician orders and California standards of practice Developing, implementing, and modifying individualized plans of care with measurable functional goals Delivering skilled therapeutic interventions, both individual and group, to restore function and reduce impairment Monitoring patient progress and adjusting treatment interventions based on clinical presentation and objective outcomes Supervising and directing Physical Therapist Assistants in accordance with California state law and professional regulations Providing appropriate oversight to rehabilitation aides, students, and volunteers as assigned Communicating evaluation findings, progress updates, and discharge outcomes to physicians, interdisciplinary team members, patients, and families Developing and instructing patients and caregivers in home exercise programs, safe use of assistive devices, orthotics, and adaptive equipment Completing timely, accurate, and compliant documentation consistent with federal, state, payer, and organizational requirements Participating in MDS assessments, care conferences, care plan development, and quarterly reviews Supporting discharge planning, transition-of-care coordination, and community reintegration recommendations Participating in quality assurance, performance improvement, utilization review, and risk management initiatives Maintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable California state practice standards Treating all residents with dignity, respect, and compassionate professionalism Physical requirements include the ability to assist with patient transfers, mobility support, repositioning, prolonged standing and walking, bending, reaching, and lifting with or without assistive devices. The ability to assist in resident evacuation during emergency situations is also required. What You Bring
Current, active, and unrestricted Physical Therapist license in the State of California - required (pending licensure considered) Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate) - required Successful completion of the National Physical Therapy Examination (NPTE) - required Current CPR/BLS certification (or willingness to obtain per facility policy) Experience in a skilled nursing, post-acute, hospital, or related medical setting - strongly preferred Training in rehabilitative and restorative therapy practices Excellent verbal and written communication skills with the ability to exercise sound clinical judgment Composure and professionalism in dynamic, fast-paced clinical environments Strong organizational skills, leadership ability, and capacity to make independent clinical decisions A team-first mindset and a genuine passion for geriatric and post-acute care
