This opportunity is ideal for therapists looking for flexibility, autonomy, and the ability to build meaningful one-on-one patient relationships without the burnout commonly found in traditional clinical settings. In this role, you'll provide individualized care in patients' homes while maintaining control over your schedule and daily workflow. Providers are trusted to work independently while receiving support from a collaborative and family-oriented leadership team when needed. Position Overview The Physical Therapist will provide skilled home health therapy services to patients throughout designated Jacksonville territories. Responsibilities include conducting evaluations, start of care assessments, reassessments, treatment planning, and discharge visits while helping patients improve mobility, strength, function, and overall quality of life. This role emphasizes relationship-based care, autonomy, and productivity-driven compensation. Key Responsibilities
Perform comprehensive physical therapy evaluations in the home setting
Complete Start of Care (SOC) assessments and OASIS documentation
Develop individualized treatment plans based on patient goals and clinical findings
Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
Conduct reassessments and discharge evaluations
Educate patients and caregivers on treatment plans, exercises, safety, and prevention strategies
Maintain accurate and timely clinical documentation within WellSky (Kinser) EMR
Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams
Manage patient caseload independently while maintaining quality patient outcomes
Work Environment & Schedule
Flexible, self-managed scheduling
No rigid clock-in/clock-out requirements
Zoned territories designed to reduce excessive driving time
Independent practice model with leadership support available when needed
Focus on one-on-one patient care and continuity of treatment
Productivity & Compensation Full-Time Compensation
$55-$58 per unit
Guaranteed minimum of 24 units per week
Most clinicians average 30-35 units weekly
Strong earning potential based on productivity
PRN Compensation
$60-$65 per unit
Additional Earnings
Referral bonuses up to $2,500
Bi-weekly pay schedule

Active Florida Physical Therapist (PT) license
Strong clinical and interpersonal communication skills
Ability to work independently in a home health environment
Reliable transportation and valid driver's license
Preferred
Home health experience preferred but not required
