Über
Conduct comprehensive assessments for patients at risk for or diagnosed with HCV.
Develop, implement, and monitor individualized care plans.
Coordinate screening, diagnostic testing, and linkage to care.
Educate patients about HCV transmission, prevention, and treatment options.
Track treatment adherence and support patients through the full course of therapy.
Serve as a liaison between patients, healthcare providers, community agencies, and pharmacies.
Facilitate access to medications, including prior authorizations and patient assistance programs.
Maintain accurate documentation in electronic health records and databases.
Participate in quality improvement activities related to HCV care and prevention.
Collaborate on outreach and prevention initiatives targeting high-risk populations.
Participate in Nursing On-Call rotation.
Other duties as assigned.
Requirements E ducation:
Graduateofanaccreditednursingprogramwithanassociateorbachelor’sinscience as a Registered Nurse.
Experience:
Experience working with underserved or high-risk populations.
Oneormoreyearsofnursingexperience,preferablyinpsychiatricnursing,emergency nursing, or substance use treatment.
Knowledge/Skills:
Knowledge of Hepatitis C prevention, transmission, and treatment guidelines.
Strong organizational, communication, and interpersonal skills.
Ability to work independently and as part of a team
Proficient with computer applications, such as Microsoft and EHR systems.
Licensures/Certifications
ValidlicensureinthestateofIowaasanRegistered Nurse.
Mandatory Reporter of Child/Dependent Adult Abuse Certification or the ability to obtain within six (6) months of hire and every three (3) years thereafter is required.
