Registered Nurse (RN) Case Manager – Home Health Job Location: Greater Twin Cities Metro Area (21-county service area) Schedule: Full-Time, Monday to Friday, 8:00 AM – 5:00 PM, 1 weekend per month, 1 on-call shift per week Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses To stand out, email Tori with a summary of your RN experience, patient population, and interest in home health care. What You'll Do:
Complete comprehensive nursing assessments and care plans. Perform OASIS visits and coordinate care across settings. Provide skilled nursing care, education, and support to patients and families. Collaborate with physicians, therapists, and interdisciplinary teams. Manage a caseload of patients in homes, assisted living, and community settings. Maintain accurate, timely documentation and regulatory compliance.
What We're Looking For:
Active Minnesota Registered Nurse (RN) license. 3+ years of RN experience, home health preferred. Strong assessment, care planning, and documentation skills. Ability to work independently in a field-based role. Reliable transportation and valid driver's license.
