About the Job About the Job: Innovative Wound Solutions, a leading healthcare organization dedicated to providing innovative and patient-centered care, is seeking a highly motivated and experienced Nurse Practitioner to work within our established Wound Care Program. Position Overview: We are looking for a dynamic and autonomous Nurse Practitioner. The successful candidate will play a crucial role in providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes. Responsibilities:Clinical Leadership:Provide direct patient care through all available channels and products. Conduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices. Monitor and evaluate patient progress, adjusting treatment plans as needed. Collaborate with interdisciplinary teams to ensure seamless patient care coordination. Chronic Wound Care Planning and Management:Develop personalized treatment plans for patients with wounds, focusing on long-term management and prevention of complications. Educate patients and families on self-management strategies and lifestyle modifications. Communication and Collaboration:Establish and maintain effective communication channels with referring providers to ensure continuity of care. Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care. Qualifications:Current and unrestricted Nurse Practitioner license in Arizona. Certification as a Nurse Practitioner with a specialization in wound care management is preferred.
