Seeking a Nurse Practitioner to provide care on our mobile health unit, delivering services at various community locations and events throughout Cumberland and Gloucester Counties. Hours and sites vary based on event schedules and service demand. Provides nursing care within the scope of a NJ licensed Nurse Practitioner
Assess the bio/psycho/social/spiritual health status of individual clients and families through history taking, data collection, and physical exam Assists staff nurses to facilitate care for walk-in patients Completes accurate documentation according to site specific standards Works in collaboration with staff, physicians, allied health professionals, and others to implement the patients plan of care Provides pharmacological care within the scope of a NJ Certified Nurse Practitioner
Participates in annual update of joint practice protocols as per NJ Board of Nursing guidelines Prescribes or make judgments regarding pharmacologic agents and therapeutic devices Dispenses/Administers/Prescribes medications according to policy & protocol Provides nurse practitioner care within the scope of a NJ Certified Nurse Practitioner
Recognizes and manages deviations from health Recognizes patients whose problem is out of scope of practice and obtain appropriate care Orders appropriate laboratory and diagnostic tests ordered Follows up on laboratory and diagnostic tests ordered Communicates with patient to arrive at mutually agreed upon plan of care to include actual and potential health problems, both medical and nursing in nature Evaluation and revision of patient plan of care is ongoing Provides health education
Informs, teaches and guides clients/families to assume responsibility for management of their own health Individualizes education to facilitate patient independence and optimize patient self care Educates patients on pharmacological agents, therapeutic devices, laboratory or diagnostic testing Promotes health education for groups of patients, families, staff, agencies, communities, and organizations pertinent to area of specialty as appropriate to program Provides referral and case management
Refers patients to internal resources according to program standards, protocol, or physician consultation Refers patients for specialty care as per program standards, protocol, or physician consultation Refers patients to primary care as appropriate Follows up on referrals according to site specific standards Case manages client care according to program standards and patients plan of care The above job description is not to be construed as a complete listing of the assignments that may be given to any employee, nor are such assignments restricted to those precisely listed in the description. EOE Requirements QUALIFICATIONS:
Candidates must be able to drive to assigned locations as scheduled.
Must have at least 2 years of experience and be able to practice independently
Unrestricted license to practice as a Nurse Practitioner from the State Board of Nurses for New Jersey and official recognition from the same State Board to practice as a Nurse Practitioner
Employee shall maintain registration including prescriptive authorization with the New Jersey Board of medical Examiners, Maintain Board Certification
Maintain Active CPR Ability to stand, swift waling, lifting, moving patients as needed
