Complete holistic assessments; develop, implement, and evaluate individualised support plans with goals that promote choice, independence, and inclusion. Administer medications, implement clinical interventions, and monitor health needs including epilepsy, dysphagia, diabetes, and complex care. Maintain accurate documentation, incident reporting, and safeguarding procedures. Liaise effectively with service users, families, advocates, MDTs, and community services. Ensure robust infection prevention, health and safety, and best practice compliance.
