*Key Responsibilities*
* *Comprehensive Assessment:* Evaluate residents across all levels of care to develop individualized treatment plans that promote mobility, safety, and independence.
* *Personalized Therapy:* Deliver high-quality physical therapy interventions, utilizing our modern equipment and gym spaces to help residents achieve their personal goals.
* *Collaborative Care:* Partner closely with our interdisciplinary team—including nursing, occupational therapy, and speech-language pathology—to ensure a holistic approach to resident wellness.
* *Documentation Excellence:* Maintain accurate and timely clinical records, ensuring all treatments and progress are documented in compliance with state and federal regulations.
* *Resident Advocacy:* Serve as a supportive resource for residents and their families, providing education on mobility techniques and home safety to ensure a seamless transition between levels of care.
*Qualifications*
* *Education:* Graduate of an accredited Physical Therapy program.
* *Licensure:* Current and valid Physical Therapist license in the state of Minnesota.
* *Experience:* Experience in skilled nursing or long-term care preferred, but we welcome passionate clinicians who thrive in a fast-paced, multi-level care environment.
* *Skills:* Strong communication skills, a "resident-first" mindset, and the ability to adapt clinical techniques to meet the needs of both active seniors and those requiring intensive rehabilitation.
