The RN Clinical Admissions Specialist at Sanford Health is responsible for coordinating and evaluating clinical admissions for patients entering our healthcare and medical services across clinics, hospitals, and senior living facilities. In this role, you will use your clinical expertise to review referrals, assess medical necessity, and determine appropriate level of care, ensuring that each patient is placed in the right setting at the right time. Working closely with physicians, care teams, case managers, and external referral sources, you will gather and review clinical information, perform nursing assessments via chart review and direct communication, and facilitate smooth transitions of care.
As an RN Clinical Admissions Specialist, you will serve as a key clinical resource for patients, families, and providers throughout the admissions process. You will clearly explain admission criteria, care options, and next steps, while demonstrating Sanford Health’s commitment to compassionate, patient-centered care. Using established guidelines, you will verify that admissions meet regulatory, payer, and organizational standards, support authorization processes with payers, and document all decisions accurately within the electronic health record. You will help identify high-risk or complex cases and collaborate with clinical leaders to develop safe, individualized plans for admission.
This position requires strong critical thinking skills, attention to detail, and the ability to prioritize multiple referrals in a fast-paced environment. You will actively participate in performance improvement initiatives, contribute feedback to enhance admission workflows, and support innovation in care coordination consistent with Sanford Health’s focus on quality and safety. You will collaborate with interdisciplinary teams across the health system, including emergency departments, inpatient units, post-acute settings, and community partners, to ensure a seamless patient experience.
