*Job Overview*
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will possess a strong understanding of anatomy and physiology, along with experience in various settings such as home health, outpatient, and inpatient care. As a Physical Therapist, you will play a crucial role in assessing patient needs, developing personalized treatment plans, and guiding patients through their rehabilitation journey to enhance their quality of life.
*Responsibilities*
* Conduct thorough evaluations of patients' physical abilities and limitations.
* Develop individualized treatment plans based on patient assessments and goals.
* Implement therapeutic interventions to improve mobility, strength, and overall function.
* Educate patients and their families on treatment protocols and home exercise programs.
* Monitor patient progress and adjust treatment plans as necessary.
* Collaborate with other healthcare professionals to ensure comprehensive patient care.
* Maintain accurate documentation of patient evaluations, treatments, and progress notes.
* Participate in discharge planning to ensure continuity of care post-treatment.
*Requirements*
* Valid Physical Therapist license in the state of practice.
* Strong knowledge of anatomy, physiology, and medical terminology.
* Experience in home health, clinic, inpatient, or outpatient settings is preferred.
* Excellent patient care skills with a compassionate approach to rehabilitation.
* Ability to develop effective discharge plans for patients transitioning from therapy services.
* Strong communication skills for interacting with patients, families, and healthcare teams.
* Commitment to ongoing professional development and staying current with industry practices.
