Licensed Clinical Social Worker

Role Description

This is a remote position for a Licensed Clinical Social Worker (LCSW). The role involves providing support to patients and families through the provision of case management, mental health assessments, and related therapeutic interventions. Responsibilities include developing treatment plans, monitoring patient progress, and coordinating care with other healthcare providers and professionals. The Licensed Clinical Social Worker will also offer guidance for behavioral health concerns and assist individuals in accessing necessary resources for their well-being.

Qualifications

• Proficiency in Social Work and Medical Social Work with the ability to assess and address patient needs
• Experience in Case Management, including planning and coordinating services for patients
• Strong knowledge of Mental Health and Behavioral Health concepts, diagnosis, and interventions
• Excellent communication, active listening, and problem-solving skills
• Ability to work independently while collaborating with healthcare teams remotely
• Relevant licensure as a Licensed Clinical Social Worker (LCSW) in the applicable state
• Familiarity with electronic health record systems is a plus
