Your Role as MDS Coordinator:
As the MDS Coordinator, you'll lead the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) process, ensuring compliance with CMS and LTCI regulations. Your work will directly impact the quality of care and outcomes for our residents.

Key Responsibilities:

• Collaborate with nursing and interdisciplinary teams to develop accurate, personalized care plans
• Ensure timely completion and transmission of RAI/MDS assessments
• Prepare and analyze Quality Measures/Indicators (QM/QI) reports
• Maintain compliance with all regulatory guidelines
• Support a culture of excellence and continuous improvement
