Position Overview

We are seeking a Full-Time PM Shift RN Charge Nurse / Med Pass Nurse to oversee clinical operations, supervise staff, and ensure safe, accurate medication administration during the evening shift.

Shift: 2:30 PM – 10:30 PM
Schedule: Full-Time | 4 x 2 rotation
Location: Lemon Grove Care Center, Lemon Grove, CA

Key Responsibilities

Charge Nurse Duties

• Provide leadership, supervision, and support to LVNs, CNAs, and unit staff on the PM shift
• Manage workflow, staffing assignments, and unit priorities
• Ensure residents receive high-quality, person-centered care
• Conduct assessments and respond promptly to changes in resident condition
• Communicate effectively with physicians, families, and interdisciplinary team members
• Monitor compliance with facility policies, procedures, and regulatory requirements
• Respond to emergencies and initiate appropriate interventions

Med Pass Responsibilities

• Accurately prepare and administer medications as ordered
• Verify resident identity and follow all safety protocols
• Document medications, treatments, and resident responses in real time
• Monitor for medication reactions or side effects
• Ensure safe medication storage, labeling, and disposal

Clinical & Resident Care

• Perform nursing procedures, including wound care and skilled treatments
• Assist with admissions, transfers, and discharges
• Support restorative and rehabilitative care programs
• Maintain accurate, compliant clinical documentation
• Ensure care plans are followed and updated

Qualifications

• Active California RN License (Required)
• Previous experience in skilled nursing or long-term care preferred
• Strong leadership, communication, and critical-thinking skills
• Ability to work independently, prioritize, and manage multiple responsibilities
• Must be fully vaccinated for COVID-19 (state requirement)
• Must pass background check and pre-employment screening
