Job Description Job Summary: Provide compassionate and dedicated nursing assistance to residents in a Skilled Nursing Facility (SNF). Work collaboratively with nursing staff, residents, families, and other healthcare team members to deliver personalized care and ensure residents' well-being. Responsibilities:
Receive and relay comprehensive reports on residents' conditions to ensure continuity of care Conduct rounds to assess residents, including new admissions and those experiencing health changes or injuries Document assessments and resident updates in accordance with facility policies Notify nursing staff of any significant changes in residents' conditions or needs Assist with direct patient care, ensuring call lights are answered promptly and residents' needs are addressed Foster positive relationships with colleagues, residents, families, and physicians to promote a supportive environment Qualifications:
Hold a valid California CNA license Demonstrate excellent communication skills and ability to work well within a team Possess strong assessment and observation skills Ability to perform routine resident assessments and document findings accurately Flexibility to work various shifts, including weekends and holidays, and to float between units as needed Prior experience in LTC or SNF settings preferred Benefits & Growth:
