Conduct thorough PT evaluations and develop individualized treatment plans Deliver evidence-based therapeutic interventions to improve mobility, strength, and functional independence Document all patient care in accordance with state, federal, and facility guidelines Collaborate with nursing, therapy, and medical teams to ensure coordinated care Educate residents and family members on exercise programs, safety, and functional strategies Participate in care conferences and contribute to ongoing plan-of-care updates
Qualifications
Valid New Mexico state license as a physical therapist (PT) Degree in physical therapy from an accredited program No experience with skilled nursing rehabilitation, supportive team to mentor and guide Strong communication skills and a passion for patient-centered care
