*Position Summary*
We are seeking a compassionate and skilled *Physical Therapist* to join our Home Care team in Moab, Utah. In this full-time role, you will evaluate, plan, and deliver individualized therapy to patients in their homes, supporting recovery, independence, and improved quality of life.
*Responsibilities*
* Conduct physical therapy assessments and develop individualized care plans.
* Deliver safe, effective, evidence-based therapy in the home setting.
* Collaborate with interdisciplinary teams, patients, and families.
* Document care according to regulatory and organizational standards.
* Educate patients and caregivers on therapy techniques and safe mobility practices.
*Qualifications*
* Current Utah Physical Therapist license required.
* Previous home health or acute care experience preferred, but not required.
* Strong communication, problem-solving, and organizational skills.
* Compassionate, patient-focused, and motivated to make a difference.
