We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in clinical pharmacy and be adept at providing exceptional patient care in a fast-paced environment. This role involves working closely with healthcare professionals to ensure the safe and effective use of medications, contributing to patient assessments, and managing acute pain management strategies in LTC and skilled facilities.
Unique position in being able to set your own hours and days of the week to work. We are a closed-door pharmacy that serves a small group of skilled facilities that require monthly medication regimen reviews. There is an onsite monthly requirement but only around 6-8 days per month. Benefits include pay based on experience and mileage. No on call, no weekends or holidays, set your own schedule.
*Duties*
* Provide comprehensive monthly medication management reviews and patient care services, ensuring optimal therapeutic outcomes.
* Collaborate with physicians and other healthcare providers to develop individualized treatment plans.
* Conduct patient assessments to evaluate medication efficacy and safety.
* Engage in managed care initiatives to enhance patient service and outcomes.
* Provide education to LTC facilities regarding residents medications, including potential side effects and interactions.
* Stay informed about current pharmacy practices, guidelines, and regulations.
*Experience*
Candidates should possess the following qualifications:
* Licensed pharmacist in Oklahoma with immunization certificate
* Previous LTC consulting experience a benefit
* Familiarity with CMS guidelines for long term care facilites.
