Become the Case Manager you always wanted to be

  • Work withinterdisciplinaryteam,guidingtreatmentplansbasedonpatientneedsand preferences.
  • Coordinatewithinterdisciplinaryteamtoestablishtentativedischargeplanandcontingency plans.
  • Participateinplanningforandtheexecutionofpatientdischarge experience.
  • Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
  • Facilitateteamconferencesweeklyandcoordinatealltreatmentplan modifications.
  • Completecasemanagementaddendumsandallrequired documentation.
  • Maintainknowledgeofregulations/standards,companypolicies/procedures,anddepartment operations.
  • Review/analyzecasemanagementreports,includingKeyCareIndicators,andplanappropriate actions.
  • Understandcommercialcontractlevels,exclusions,payorrequirements,andrecertification needs.
  • Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
  • Meetwithpatient/familyperPatientArrivalandInitialVisitStandardwithin24hrs.of admission.
  • Performassessmentofgoalsandcompletecasemanagementaddendumwithin48hoursof admission.
  • Educatepatient/familyonrehabilitationandCaseManagerrole;establishcommunication plan.
  • Scheduleandfacilitatefamilyconferencesas needed.
  • Assistpatientwithtimelyprocuring/planningofresourcestoavoiddischargedelaysor issues.
  • Monitorcompliancewithregulationsfororthoticsandprostheticsorderingand payment.
  • Makeappropriate/timelyreferrals,includingdocumentationtopostdischarge providers/physician
  • Ensureaccuracyofdischargeandpayor-relatedinformationinthepatient record
  • Participateinutilizationreviewprocess:datacollection,trendreview,andresolution actions.
  • Participateincasemanagementon-callscheduleas needed.

Qualifications

  • Licenseor Certification:
    • Must be qualified to independently complete an assessment within the scope of practiceofhis/herdiscipline(forexample,RN,SW,OT,PT,ST,andRehabilitation Counseling).
    • Iflicensureisrequiredforonesdisciplinewithinthestate,individualmustholdan active license.
    • MustmeeteligibilityrequirementsforCCM®orACMcertificationuponentryintothis position OR within two years of entry into the position.
    • CCM®orACMcertificationrequiredORmustbeobtainedwithintwoyearsofbeing placed in the Case Manager II position.

·Minimum Qualifications:

    • For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirementforlicensureorcertificationisadiplomaratherthanan associate degree.
    • Forallothereligiblelicensedorcertifiedhealthcareprofessionals,mustpossessa minimum of a bachelors degree and graduate degree is preferred.
    • 2yearsofrehabilitationexperience preferred.
