*Responsibilities:*
* Evaluate and assess patients’ physical conditions and home environments
* Develop individualized treatment plans to improve mobility, strength, and function
* Provide hands-on therapy, therapeutic exercise, and patient/family education
* Collaborate with nurses, aides, and other team members to coordinate care
* Maintain accurate documentation (EMR experience a plus, OASIS preferred)
* Supervise PTAs and participate in case conferences and in-service education
*Qualifications:*
* Current CA Physical Therapist license
* Graduate of an accredited PT program
* CPR certification
* Valid driver’s license, insured and reliable vehicle
* Minimum 2 years PT experience (home health preferred but not required)
* Strong communication and interpersonal skills
* Knowledge of PDGM Medicare model a plus
