Physical Therapist

In this on-site role, you will assess functional status, develop individualized treatment plans, and deliver therapeutic interventions to enhance mobility, safety, and quality of life for a diverse post-acute and geriatric patient population. You will collaborate closely with an interdisciplinary team, document care using EMR systems, and educate patients and families on home programs.
