In this dynamic role, you will evaluate strength, balance, gait, transfers, pain, and functional abilities; prescribe therapeutic exercise; provide gait and transfer training; recommend and train patients in the use of assistive devices and durable medical equipment; and design home exercise programs that empower patients and families. You will educate patients and caregivers on fall prevention, energy conservation, safe mobility within the home, and management of chronic conditions affecting physical function. You will document all care in an electronic medical record in accordance with regulatory, payer, and agency standards, ensuring accurate, timely, and complete clinical notes, care plans, and progress reports.
As a float clinician, you will adapt quickly to changing caseloads and geographies, covering for team members who are on leave or when census needs shift. You will demonstrate strong time management and organizational skills to plan daily routes, prioritize urgent visits, and meet productivity and quality expectations. You will communicate proactively with supervisors and interdisciplinary team members regarding changes in patient status, safety concerns, or the need for additional services or equipment.
