As a Nurse Practitioner with Senior Community Care, you will perform detailed histories and physical exams, order and interpret appropriate diagnostic tests, and develop individualized care plans that emphasize prevention, safety, and quality of life. You will prescribe and manage medications, monitor treatment effectiveness, and adjust care plans based on clinical findings and participant goals. You will collaborate daily with physicians, nurses, social workers, therapists, and other healthcare professionals to coordinate services, close care gaps, and ensure a seamless experience for each participant and their family.
You will play a key role in educating participants and caregivers about diagnoses, medications, and self-management strategies, using clear, respectful communication suited to the needs of older adults. Your practice will emphasize integrity, respect, and compassion, reflecting our commitment to high-quality care for elderly populations. You will support transitions of care, participate in care conferences, and contribute to quality improvement initiatives aimed at enhancing outcomes and service delivery across our program.
The ideal candidate is comfortable working independently within a collaborative model, confident in geriatric and adult primary care, and committed to evidence-based practice. Strong documentation skills and proficient use of Electronic Health Records are essential, along with adherence to organizational policies, clinical protocols, and regulatory standards. You will be expected to maintain current licensure and certifications, participate in ongoing professional development, and engage in regular feedback and performance review processes.
