Physical Therapist

Altru Rehabilitation Hospital, part of Altru Health System, is seeking a dedicated Physical Therapist to join our Healthcare & Medical Services team in the Rehabilitation department. In this role, you will provide evidence-based physical therapy evaluation and treatment to patients with a variety of orthopedic, neurological, cardiopulmonary, post-surgical, and medically complex conditions across the continuum of care. You will perform comprehensive assessments, develop individualized treatment plans, and deliver interventions focused on restoring function, improving mobility, reducing pain, and optimizing patients’ independence and quality of life. Working within Altru’s collaborative, community-focused culture, you will partner closely with physicians, nurses, occupational and speech therapists, case managers, and other members of an interdisciplinary care team to coordinate patient-centered care and seamless discharge planning. Daily responsibilities include conducting detailed patient examinations, establishing functional goals in collaboration with patients and families, implementing therapeutic exercise, manual therapy, gait and balance training, neuromuscular re-education, and use of therapeutic modalities in accordance with established clinical guidelines and safety standards. You will regularly reassess patient progress, modify plans of care, and provide clear, timely documentation in the electronic medical record to support quality, continuity, and regulatory compliance. Education and coaching are integral parts of the role; you will teach patients and caregivers home exercise programs, safe mobility strategies, fall prevention techniques, and use of assistive devices, while promoting long-term wellness and self-management.
