As an MDS/Resident Assessment Coordinator , you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Mountain View by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

• As a minimum, an unencumbered State of South Dakota R.N. License;

• C.P.R. Certification;

• Prior experience as a MDS Coordinator preferred, but we are willing to train if candidate meets all other qualifications ;

• Three (3) years of experience as an R.N. in a Skilled Nursing Facility setting;

• One (1) year of experience as a C.N.A. is preferable.

  NOTE: MDS/Resident Assessment Coordinator is a rewarding career and one that is very much in demand. If you are not currently a certified MDS Coordinator, but have at least three (3) years experience as an RN in a skilled nursing environment and would like to learn to be an MDS Coordinator, we may be willing to provide training.
