**Job Summary and Responsibilities**
As our Wound Ostomy Nurse, you will connect our hospital directly to the community, delivering specialized in-home care that empowers patients to heal comfortably and effectively.
Every day you will expertly manage in-home patient visits, performing advanced treatments like PICC line care, wound vacs, and infusions, while educating patients and families on complex conditions and treatments.
To be successful in this role you must be an exceptionally organized, independent professional, adept at managing complex schedules, patient care, and technology-driven documentation. Your deep commitment to patient engagement and advocacy will make you a vital defense against declining health, fostering a profound impact on individual well-being.
+ Must be able to identify, manage, and treat specific skin disorders, primary and secondary lesions, and decubitus ulcers. Provide primary skin care to residents under the medical direction and supervision of the residents' attending physicians, or the Director of Nursing Services, with an emphasis on treatment and therapy of skin disorders.
+ Prepares and administers medications as ordered by the physician in accordance with nursing standards and facility policies. Signs, dates, and performs all charting and record keeping in accordance with established policies and procedures.
+ Provide demonstrations on dressing techniques to educate and orient in-services nurses; give hands-on instruction regarding the principles of aseptic technique.
+ Understands, complies with and promotes all rules regarding resident rights.
+ Provide a list of patients with Nosocomial pressure ulcers which includes patient's name, site, state, support surface and dates of evaluation to the Director of Nurses.
+ While performing daily rounds, assist in delivery of patient care to accomplish wound care tasks. Ensure that residents with decubitus ulcers received appropriate prophylaxis and treatment such as daily inspection, turning and activity, a well-planned diet, and maintaining a clean, dry bed.

Required
+ Bachelor of Science in Nursing
+ One year Long Term Care nursing experience, upon hire
+ Registered Nurse: TX, upon hire and
+ Basic Life Support - CPR, within 14 - days

**Pay Range**
$35.62 - $52.99 /hour
