Responsibilities
* Communication skills to work with nursing staff for implementation of care plan
* Oversees care plan meeting with resident and family
* Assures that care plans are keep current and reflective
* Verifies that assessments transmitted to database with necessary corrections
* Reviews of the MDS for accuracy for any trends that need education
* Monitors documentation of Medicare and managed care resident and educate as needed
* Monitoring of QM with emphasis on those impacting five-star rating
* Experience in CMI for best outcomes and guidelines for documentation required
* Knowledge of restorative programs and implementation
* Participation in Quality Improvement program in the facility
* Have 2-4 years work experience in MDS and long term care
* Point Click Care experience preferred
Qualifications
* RN degree from an accredited college (minimum ASN)
* Currently registered in the state
* Have AANAC certification
* Must have current CPR certification
