As a Physical Therapist with PruittHealth, you will complete thorough assessments, set realistic and measurable goals in collaboration with patients, families, and the interdisciplinary care team, and regularly reassess progress to adjust treatment plans. You will provide therapeutic exercises, gait and balance training, functional mobility training, neuromuscular re-education, and patient and caregiver education tailored to each patient’s abilities and goals. You will document all evaluations, treatments, and patient responses accurately and timely within our electronic medical record system, ensuring compliance with regulatory, payer, and professional practice standards.
You will work closely with physicians, nurses, occupational and speech therapists, social workers, and other team members to coordinate patient-centered care plans and safe discharges or transitions of care. You will contribute to fall-prevention initiatives, equipment and assistive device recommendations, and home safety planning when appropriate. As a key clinical resource, you will educate patients, families, and caregivers on safe mobility techniques, proper use of assistive devices, energy conservation, and home exercise programs to promote long-term functional independence.
