*Responsibilities Include:*
The RN MDS Coordinator will be responsible for updating and maintaining care plans according to state and federal regulations. The MDS Coordinator will also need to work with the interdisciplinary team to provide education to the nursing team related to MDS changes or any issues with MDS accuracy. Meets with the DON and NHA to review and keep abreast of any changes, issues etc.
*Qualifications*:
MDS Coordinator experience
SNF/LTC experience
RN License in CT Preferred
