Occupational Therapist

CenterWell Home Health is seeking a compassionate Occupational Therapist to provide skilled, in-home therapy services to adult and geriatric patients. In this role, you will evaluate functional abilities, develop person-centered treatment plans, and deliver evidence-based interventions that promote safety, independence, and quality of life at home. You will collaborate closely with an interdisciplinary care team, educate patients and caregivers, document services in accordance with regulatory standards, and contribute to continuous quality improvement in a supportive, growth-focused culture.
