RN Case Manager

Good Samaritan Health Center in Atlanta seeks a compassionate RN Case Manager to coordinate holistic, patient-centered care for underserved individuals and families. In this role, you’ll conduct clinical assessments, develop and implement care plans, provide education and advocacy, and collaborate closely with an interdisciplinary team across primary care, dental, behavioral health, and wellness services.
