We are currently seeking an experienced:

Palliative Care RN

Per Diem - Flexible Schedule

(Covering Harnett County & surrounding areas)

Job Summary:

• Review referrals

• Provides expert, individualized coordination of care to patients and families

• Completes a comprehensive physical and psychosocial assessment of the patient and family, to identify need and areas for intervention

• Ensures appropriate referrals to specialists when indicated and supports adherence to treatment plan, including physician visits for evaluation and ongoing management.

• Assists patients in accessing appropriate services and resources in the community to improve their overall health and well-being.

• Assists Nurse Practitioners in day-to-day program tasks and tracking palliative referrals.

• Alleviates pain, fatigue, nausea, and other distressing symptoms.

• Accurately documents patient assessments, interventions, responses, needs, goals, and teaching in the EHR.

Job Requirements:

• Requires current RN licensure within the state of branch location and three years of clinical nursing experience, preferably in Home Care, Hospice, or Skilled Nursing facility setting.

• Successfully completed nursing education from an approved school of nursing.

• Must be able to independently organize work, communicate patient needs, and provide patient education and coordinate patient visit schedule(s).

• Must accept travel and working conditions as assigned.

• Must possess effective communication skills (oral and written), as well as CPR certification. Minimum of twelve hours of continuing education in a twelve-month period.

• Must have a valid driver's license.
