Are you a Physical Therapist (PT) looking for a flexible role with purpose and real impact? At Tennessee Quality Care, we are seeking a compassionate PT to join our Home Health team on a PRN basis.

What You'll Do
  • Provide physical therapy services according to a physician's plan of care and state practice guidelines
  • Evaluate patient function and assist in developing individualized care plans
  • Educate patients, families, and caregivers on treatment plans and therapeutic activities
  • Translate prescribed exercises into functional activities and activities of daily living
  • Recommend modifications to equipment and home environments to support patient safety and independence
  • Collaborate with physicians and interdisciplinary team members to coordinate care and monitor patient progress
  • Document clinical notes, progress reports, and summaries in a timely and accurate manner
  • Support and motivate patients and families throughout the rehabilitation process
  • Supervise Physical Therapist Assistants in accordance with company policy and state regulations
  • Maintain accurate clinical records and track patient goals and outcomes
Qualifications
  • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
  • Current Physical Therapist (PT) license in the state of practice
  • At least two (2) years of experience in an acute care or similar setting (preferred)
  • Current CPR and First Aid certification
  • Reliable transportation, valid driver's license, and current automobile insurance
