Working collaboratively with an interdisciplinary care team, you will communicate regularly with physicians, nurses, case managers, and other rehabilitation professionals to coordinate care and support safe, effective discharge planning. You will educate patients and their families on home exercise programs, injury prevention, and strategies to improve independence, tailoring your teaching to each patient’s goals and abilities.

You will demonstrate empathy and respect for patients from diverse backgrounds, maintain accurate and timely clinical documentation in the electronic medical record, and adhere strictly to all regulatory, safety, and infection control standards. Responsibilities include conducting physical therapy assessments, setting measurable functional goals, implementing therapeutic exercises and manual therapy techniques, utilizing modalities as appropriate, and regularly reassessing progress to adjust treatment plans. You will help mentor and support physical therapist assistants and rehabilitation technicians as appropriate, and may assist with student clinical education. Flexibility is required to work a part-time schedule that may include weekdays, occasional weekends, or holiday rotations based on department needs.
