*Clinical Pharmacist Job Description*
Responsible for providing direct patient care and medication management services in a federally funded health center. Duties include chronic disease management, medication reconciliation, patient counseling, collaboration with interdisciplinary care teams, documentation in the EHR, support of quality improvement and population health initiatives, and compliance with pharmacy regulations and 340B requirements. Requires PharmD and active state pharmacist licensure; ambulatory care or FQHC experience preferred.
*Essential Duties and Responsibilities*
* Provide *comprehensive medication management* for patients with chronic and acute conditions, including diabetes, hypertension, asthma/COPD, depression, HIV, and other common primary care needs.
* Conduct *medication reconciliation*, medication therapy management, and comprehensive medication reviews.
* Identify, prevent, and resolve *medication-related problems*, including drug interactions, duplications, contraindications, adverse effects, and adherence barriers.
* Educate patients and caregivers on medication use, side effects, disease states, lifestyle changes, and self-management strategies.
* Collaborate with physicians, nurse practitioners, physician assistants, nurses, care coordinators, and behavioral health staff to develop and adjust patient-specific treatment plans.
* Document clinical interventions, recommendations, and outcomes accurately in the electronic health record.
* Participate in *population health*, preventive care, and quality improvement programs designed to improve clinical outcomes and reduce avoidable utilization.
* Support transitions of care by reviewing discharge medications, reconciling medication lists, and helping reduce readmissions.
* Assist with formulary management, prior authorizations, therapeutic substitutions, and non-formulary reviews where applicable.
* Administer immunizations and provide protocol-based services permitted under state law and collaborative practice agreements.
* Serve as a medication resource for providers and staff by answering drug information questions and supporting safe prescribing practices.
* Assist with oversight of pharmacy operations, including dispensing workflows, inventory controls, medication storage, and compliance with controlled substance requirements, if applicable to the site.
* Support *340B program* activities and help ensure compliance with applicable pharmacy and federal program requirements.
* Participate in staff training, student/resident precepting, interdisciplinary meetings, and performance improvement activities.

*Qualifications*
*PharmD- required*
* Doctor of Pharmacy (*PharmD*) from an accredited school of pharmacy required; BS Pharmacy may be accepted where legally sufficient and consistent with organizational requirements.
* Current, unrestricted pharmacist license in the applicable state required.
* Completion of a PGY1 residency or equivalent ambulatory/clinical pharmacy experience preferred.
* Experience in *primary care, ambulatory care, community health, public health, or underserved populations* preferred.
* Knowledge of federal and state pharmacy law, controlled substance requirements, and documentation standards required.
* Experience with *electronic health records*, quality metrics, and team-based care models preferred.
* Immunization certification, BLS/CPR, and other state-required certifications may be required.
* Bilingual skills may be preferred depending on patient population.

* Ability to maintain confidentiality and comply with HIPAA and other applicable regulations.
* Strong organizational skills and the ability to balance clinical, operational, and documentation responsibilities.
*Working Conditions*
* Work is performed in a community health center, outpatient clinic, or integrated primary care setting.
* May require standing, walking, and occasional lifting of up to 25 pounds.
* May require travel between clinic sites.
* Exposure to routine clinical risks, including contact with patients with infectious diseases.

Pay: $55.00 - $60.00 per hour
