Wapakoneta Manor is a 73-bed skilled nursing facility with strong growth potential, and we’re looking for a *Physical Therapist (PT)* to join our team.

*Position Summary:*
The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and delivering high-quality therapy services to help residents regain strength, mobility, and independence.
*Responsibilities:*
* Evaluate and assess residents’ physical therapy needs
* Develop and implement individualized treatment plans
* Provide therapy services in accordance with physician orders
* Collaborate with interdisciplinary team members
* Maintain accurate and timely documentation
* Ensure compliance with all regulatory standards
*Qualifications:*
* Licensed Physical Therapist in the state of Ohio
* Strong clinical and communication skills
* Experience in skilled nursing or long-term care preferred, but not required
* Ability to work independently and as part of a team
