Carle Health is seeking a Substance Use Disorder Outreach Coordinator to support our mission of improving community health outcomes through compassionate, evidence-based care. In this role, you will serve as a key liaison between Carle Health, community partners, and individuals affected by substance use disorders, helping people access treatment, recovery resources, and supportive services. Working within the Community & Social Services department, you will design and implement outreach strategies that increase awareness of substance use disorder, reduce stigma, and promote early intervention. The Substance Use Disorder Outreach Coordinator will conduct community-based outreach in neighborhoods, clinics, shelters, and partner agencies to identify individuals at risk or currently experiencing substance use challenges. You will provide screenings, brief interventions, and referrals to appropriate Carle Health programs or community-based treatment providers. You will collaborate closely with clinical teams, social workers, behavioral health professionals, and community organizations to ensure a warm handoff and coordinated care for individuals and families. In addition to direct outreach, you will plan and facilitate educational sessions, workshops, and community events focused on substance use prevention, harm reduction, and recovery support. You will develop culturally sensitive educational materials, track participation, and gather feedback to refine programming. The role involves maintaining accurate documentation of outreach contacts, referrals, and outcomes in accordance with Carle Health policies and regulatory requirements, while protecting patient privacy and confidentiality. The ideal candidate brings knowledge of substance use disorders, recovery models, and community resources, along with strong communication and engagement skills. You should be comfortable working with diverse populations, including individuals who may be in crisis or experiencing homelessness, mental health conditions, or justice involvement. You will use motivational interviewing and trauma-informed approaches to build trust and encourage individuals to seek help, always prioritizing safety and dignity. This position requires excellent organizational skills and the ability to manage multiple community relationships. You will establish and maintain partnerships with local health departments, social service agencies, schools, law enforcement, and advocacy groups to strengthen referral networks and enhance access to care. You will regularly report outreach activities, trends, and barriers to leadership, contributing to data-driven improvements in Carle Health’s community-based substance use programs.
