Physical Therapist

Trilogy Health Services, a leader in senior living and long-term healthcare services, is seeking a compassionate and skilled Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide personalized, evidence-based physical therapy services to older adults and long-term care residents, helping them maintain or restore mobility, independence, and quality of life. Working in a collaborative, patient-centered environment, you will perform comprehensive evaluations, design individualized treatment plans, and deliver one-on-one and group interventions focused on strength, balance, gait, functional mobility, pain management, and fall prevention. You will document all care using electronic medical records, track meaningful functional outcomes, and regularly update treatment plans based on resident progress and clinical judgment. As a key member of an interdisciplinary team, you will partner closely with nurses, physicians, occupational and speech therapists, social workers, and support staff to coordinate care that reflects each resident’s goals, medical status, and personal preferences. You will educate residents and their families on home exercise programs, safe mobility techniques, and the use of assistive devices, empowering them to participate actively in their rehabilitation journey.
