Kingston Health Center of Vermilion, a leader in assisted living, rehabilitation, and memory care in Vermilion, OH, is seeking a dedicated MDS Coordinator to join our Healthcare & Medical Services team. As an MDS Coordinator at Kingston Healthcare, you will play a key role in ensuring accurate, timely, and comprehensive resident assessments that support exceptional clinical outcomes, quality measures, and appropriate reimbursement. Working within a collaborative, compassionate, and patient-centered culture, you will partner closely with nursing, therapy, and other interdisciplinary team members to develop and maintain individualized care plans that enrich the lives of our residents.
In this role, you will be responsible for coordinating and completing the full RAI/MDS 3.0 assessment process for residents in our skilled nursing and long-term care programs. You will gather and validate clinical data from multiple disciplines, enter and review MDS information for accuracy, and ensure all assessments and related documentation meet federal, state, and facility standards. You will monitor key quality indicators, case mix index, and reimbursement-related data to help the organization optimize both resident care and financial integrity. The MDS Coordinator will communicate regularly with physicians, therapists, social workers, and families to ensure that assessed needs are accurately reflected in resident-centered care plans and that changes in condition are promptly captured and addressed.
The MDS Coordinator will provide guidance and education to nursing and interdisciplinary staff on documentation practices that support accurate coding and regulatory compliance. You will help drive quality improvement initiatives by analyzing assessment and outcome data, identifying trends, and collaborating on action plans to enhance resident safety, satisfaction, and functional outcomes. You will also participate in care conferences, audits, and survey preparation activities, helping Kingston Healthcare maintain readiness for regulatory reviews and uphold its reputation for delivering outstanding care.

The ideal candidate will be a licensed Registered Nurse in Ohio with experience in skilled nursing or long-term care, strong knowledge of MDS 3.0 and the RAI process, and familiarity with Medicare and Medicaid reimbursement guidelines. Attention to detail, excellent organizational skills, and the ability to work both independently and collaboratively in a fast-paced environment are essential.
