CenterWell Home Health is seeking a dedicated Physical Therapist to deliver high-quality, in-home rehabilitation services to adult and geriatric patients. In this role, you will provide one-on-one care in the patient’s home, focusing on restoring function, improving mobility and safety, and enhancing overall quality of life. Drawing on your clinical expertise, you will perform comprehensive evaluations, develop individualized plans of care, and implement evidence-based therapeutic interventions tailored to each patient’s unique needs, goals, and home environment.
As a Physical Therapist with CenterWell Home Health, you will regularly assess strength, balance, endurance, gait, and functional mobility; prescribe therapeutic exercises; provide gait and transfer training; and recommend assistive devices or home modifications to promote independence and fall prevention. You will educate patients and caregivers on home exercise programs, safe mobility techniques, energy conservation, and strategies to manage pain and chronic conditions. Accurate, timely documentation in the electronic health record will be essential to ensure regulatory compliance, communicate progress, and support high-quality clinical decision-making.
You will collaborate closely with an interdisciplinary team that may include nurses, occupational therapists, speech-language pathologists, medical social workers, and physicians to coordinate care and optimize outcomes. Regular communication with referring providers, participation in case conferences, and ongoing reassessment of patient goals will be central to your work. You will manage your own schedule and daily caseload within an assigned territory, traveling between patient homes and adapting your care to varied living situations while upholding strict infection control and safety standards.
