Oak Street Health is seeking a Primary Care Physician (Primary Care Physician-OSH) to join our Healthcare & Medical Services team dedicated to improving health outcomes for adults on Medicare in underserved communities. As a physician at Oak Street Health, you will provide comprehensive primary care grounded in a preventative, value-based model, delivering high-quality, patient-centered care in a supportive, team-based environment. In this role, you will manage a panel of adult patients, focusing on prevention, early intervention, and effective chronic disease management. You will conduct thorough patient evaluations, perform routine and problem-focused visits, order and interpret diagnostic tests, and develop individualized care plans that emphasize prevention, wellness, and evidence-based treatment. You will coordinate care with an interdisciplinary team, including nurses, medical assistants, care managers, social workers, and other specialists, to ensure patients receive seamless, holistic care that addresses both medical and social determinants of health. You will spend ample time with each patient to build strong, trusting relationships, educate them on their conditions and treatment options, and partner with them in shared decision-making. A key part of your work will involve proactively managing high-risk and medically complex patients, closing care gaps, tracking outcomes, and using data and population health tools to drive continuous improvement in quality and patient experience. The Primary Care Physician at Oak Street Health also plays an important role in care coordination and transitions of care, including follow-up after hospitalizations, emergency department visits, or specialty consultations. You will collaborate closely with external providers and community resources to ensure continuity, avoid unnecessary hospital utilization, and support patients and their families through changes in health status. Aligned with Oak Street Health’s patient-centered, inclusive, and collaborative culture, you will participate in team huddles, case reviews, quality improvement initiatives, and clinical best-practice development. You will contribute to a positive, supportive clinic environment that values innovation, respect, diversity, and continuous learning. Responsibilities include providing direct primary care to adults primarily on Medicare; conducting comprehensive annual wellness visits and chronic disease follow-up; managing common primary care conditions and coordinating specialty referrals when appropriate; utilizing electronic health records effectively for documentation, ordering, and quality tracking; and adhering to all regulatory, compliance, and ethical standards. The ideal candidate is board certified or board eligible in Internal Medicine or Family Medicine, committed to health equity and serving underserved populations, comfortable working in a value-based care model, and eager to collaborate with a multidisciplinary team. Strong communication skills, cultural humility, adaptability, and a passion for preventative care are essential. This is a full-time, clinic-based role with a primarily on-site work environment in an Oak Street Health center.
