We are seeking a dedicated and compassionate Physical Therapist to join our team. *Responsibilities*
* Conduct thorough assessments of patients' physical abilities and limitations.
* Develop individualized care plans based on patient evaluations and goals.
* Provide one-on-one, hands-on therapy to a diverse population, utilizing a variety of treatment techniques.
* Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
* Educate patients and their families on exercises, home care strategies, and lifestyle modifications to support recovery.
* Collaborate with healthcare professionals across various systems to provide comprehensive patient care.
* Maintain accurate documentation of patient progress, treatment plans, and outcomes in compliance with regulatory standards.
*Skills*
* Think critically and problem-solve with a focus on achieving optimal patient outcomes.
* Strong knowledge of care plan development and implementation.
* Excellent communication skills for effective interaction with patients, families, and healthcare teams.
* Participate in team meetings and collaborate with other healthcare professionals to enhance patient care.
* Stay current with the latest trends and research in physical therapy, and apply them in practice.
