PruittHealth is seeking a dedicated Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will provide high quality, patient-centered occupational therapy services to individuals across a variety of care settings, including skilled nursing, rehabilitation, home health, and hospice. You will perform comprehensive assessments, develop individualized treatment plans, and deliver evidence-based interventions designed to maximize functional independence, safety, and quality of life for patients of diverse ages and conditions. As an Occupational Therapist with PruittHealth, you will collaborate closely with an interdisciplinary team of physicians, nurses, rehabilitation professionals, social workers, and caregivers to coordinate and optimize patient care. You will evaluate patients’ physical, cognitive, and psychosocial needs; establish realistic, measurable goals; and implement therapeutic activities, ADL retraining, therapeutic exercises, and adaptive equipment training to support recovery and functional improvement. You will regularly reassess patient progress, modify treatment plans as needed, and provide clear documentation in accordance with professional standards, regulatory requirements, and PruittHealth policies. You will also educate patients, families, and caregivers on home exercise programs, adaptive techniques, safety strategies, and the use of assistive devices to promote independence in the least restrictive environment. In collaboration with case management and nursing staff, you will contribute to discharge planning and make recommendations regarding continued therapy, home modifications, and community resources.
