PruittHealth is seeking a dedicated and compassionate Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide high-quality, evidence-based rehabilitation services to patients across a continuum of care, including skilled nursing, outpatient, and home health settings, depending on organizational needs. You will work closely with an interdisciplinary care team to help patients restore function, improve mobility, manage pain, and achieve the highest possible level of independence and quality of life. As a Physical Therapist with PruittHealth, you will perform comprehensive evaluations, develop individualized plans of care, and deliver therapeutic interventions aligned with best clinical practices and regulatory standards. You will educate patients, families, and caregivers on home exercise programs, safety strategies, and wellness practices to support long-term success. You will also regularly reassess patient progress, modify treatment plans, and document all services accurately and timely within our electronic medical record system. You will collaborate daily with physicians, nurses, occupational and speech therapists, social workers, and other team members to coordinate care, participate in care plan meetings, and contribute to discharge planning. You will advocate for patient needs, communicate clearly and professionally, and uphold PruittHealth’s commitment to dignity, respect, and patient-centered care.
