Avera Health is seeking a compassionate and patient-centered Occupational Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based occupational therapy services to patients across the continuum of care, helping individuals of all ages regain function, maximize independence, and improve their quality of life. Working in collaboration with an interdisciplinary team of physicians, nurses, rehabilitation professionals, and care coordinators, you will evaluate patients’ functional abilities, develop individualized treatment plans, and implement therapeutic interventions focused on activities of daily living, mobility, cognition, and adaptive strategies.
As an Occupational Therapist with Avera Health, you will perform comprehensive assessments, set measurable goals in partnership with patients and families, and regularly reassess progress to refine care plans. You will provide hands-on treatment, patient and family education, home exercise and activity programs, and recommendations for adaptive equipment, splinting, and environmental modifications to support safe, independent living. You will document care using electronic medical records in accordance with regulatory and payer requirements, ensuring accurate, timely, and professional communication of patient status and outcomes.

You will coordinate closely with other members of the care team to support safe discharges, smooth transitions of care, and continuity of services across hospital, clinic, and post-acute settings. Candidates should bring strong clinical reasoning skills, a commitment to evidence-based practice, and the ability to tailor interventions to diverse patient populations, including those recovering from surgery, stroke, neurological conditions, orthopedic injuries, chronic illness, and age-related functional decline. Excellent communication, organization, and time-management skills are essential, as is the ability to build trusting relationships with patients, families, and colleagues.
