Occupational Therapist

CenterWell Home Health is seeking a compassionate, licensed Occupational Therapist to provide high-quality, in-home therapy services to adult and senior patients. In this role, you will travel to patients’ homes to evaluate functional abilities, develop individualized treatment plans, and deliver evidence-based interventions that promote independence, safety, and quality of life. You will assess patients’ physical, cognitive, and psychosocial needs, recommend adaptive equipment and home modifications, and educate patients and caregivers on safe techniques for daily activities such as bathing, dressing, grooming, and mobility. Working closely with an interdisciplinary team of nurses, therapists, and physicians, you will coordinate care, monitor progress toward goals, and update plans of care in accordance with clinical standards and physician orders. You will document all visits and patient responses accurately and promptly in the electronic health record, ensuring regulatory compliance and supporting continuity of care.
