In this role, you will manage a dedicated panel of adult and geriatric patients, providing routine primary care, acute care, and chronic disease management in a value-based care environment. You will conduct thorough preventive health visits, develop individualized care plans, and coordinate closely with an interdisciplinary team that may include nurses, medical assistants, care managers, social workers, behavioral health specialists, and clinical pharmacists. Using data-driven insights and evidence-based guidelines, you will proactively identify care gaps, close them through targeted interventions, and partner with patients and families to support adherence to treatment plans.
You will practice in a supportive, team-based setting that emphasizes longer patient visits, continuity of care, and strong relationships between providers and patients. Your responsibilities will include performing comprehensive assessments, ordering and interpreting diagnostic tests, prescribing medications, and making appropriate referrals to specialty care when necessary. You will document all encounters accurately and efficiently in an electronic health record system and participate in quality initiatives designed to improve clinical outcomes, patient experience, and operational performance.
