In this role, you will deliver high-quality outpatient primary care through in-person visits and, when appropriate, telehealth encounters. You will perform routine wellness visits, medication management, and preventive screenings, as well as manage complex comorbidities common in older adult populations such as diabetes, hypertension, COPD, heart disease, and other chronic illnesses. You will utilize evidence-based guidelines, population health tools, and data analytics to identify care gaps, reduce avoidable hospitalizations, and promote proactive, preventative care.
Aligned with Oak Street Health’s patient-centered, inclusive, and collaborative culture, you will participate actively in team huddles, case conferences, and quality improvement initiatives. You will contribute to a supportive, innovative environment where clinical staff work together to design and refine care models that enhance accessibility and patient experience. You will engage families and caregivers, communicate clearly and empathetically with diverse patient populations, and advocate for resources that address social determinants of health.
You will be responsible for accurate and timely documentation in the electronic health record, appropriate use of diagnostics and referrals, and adherence to all regulatory and compliance requirements, including Medicare and value-based care standards. You will help drive performance on quality metrics, patient satisfaction, and clinical outcomes, while maintaining a manageable schedule that supports thorough, unhurried patient visits.
