You will be responsible for implementing individualized plans of care, guiding patients through therapeutic exercises and activities, and utilizing appropriate assistive devices and adaptive equipment. You will monitor and document patient progress using electronic medical records, report changes in condition to the supervising Physical Therapist, and assist in adjusting treatment plans to ensure optimal clinical outcomes. Education is a key component of this position; you will instruct patients and caregivers in home exercise programs, safety techniques, and strategies to maximize independence in daily activities.
This role is highly interactive and patient-centered, requiring excellent communication skills, professionalism, and empathy when working with older adults and their families. You will collaborate closely with an interdisciplinary healthcare team, including physical therapists, occupational therapists, physicians, nurses, and other clinicians, to support coordinated and comprehensive care. You will also uphold all regulatory, ethical, and organizational standards, adhering to infection control, safety protocols, and documentation requirements.
