*Job Summary:*
The role of the Physical Therapist at Active Therapy is responsible for evaluation, planning, directing and administering of physical therapy in the assisted living, independent living and memory care. This role will work closely with patients, their families, and the Assisted Living Community’s staff to develop individualized treatment plans, plans of care and maintenance programs to aide in the highest level of functionality. We are a new and rapidly growing therapy program serving aging adults in assisted living, independent living and memory care communities.
*Essential Functions:*
* Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
* Directs the assistance of support staff to achieve patients functional goals
* Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.
* Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community.
* Communicates effectively with other professional and support staff to achieve positive patient outcomes
* Evaluates the patient’s functional abilities/disabilities including range of motion and muscle testing. Through assessment and testing procedures establishes a patient’s functional baseline measurement to be used throughout treatment as a gauge of progress and physical improvement.
* Consults with social services/case management, home health agencies, and the patients’ families regarding discharge planning; Including but not limited to: the regimen of home treatment, the patient’s physical level of independence, and level of nursing care needed
* Documents the patient’s treatment and progress in patient’s indicating the condition in relation to their established medical goals
* Assists in Quality Improvement monitoring and analysis
* Provides ongoing communication to patients, families and assisted living community staff
* Complies with federal and state regulations, along with follows organizational policies
* Other duties as assigned
*Certifications, Licenses, Registrations and Other Requirements:*
* Graduated from an accredited Physical Therapy program
* Valid Physical Therapy License in the state of Minnesota
* Certified in Basic Life Support
* Minimum of one year of experience
* Must pass all applicable background checks and health screens as required by the Minnesota Department of Health
*Physical Demands:*
* Must be able to travel to provide support to multiple communities in Minnesota
* Must be able to provide hands-on care as needed to patients. This may include lifting, bending, pushing, and twisting to assist patients.
Job Types: Full-time, Part-time
Pay: $40.00 - $50.00 per hour
Expected hours: 25 – 40 per week
